SRG Minutes/Budget Form

The SRG Minutes/Budget Form is signed by the SRO and the Chairperson of the meeting (as designated by the SRO) to confirm that the meeting was held and to describe what was reviewed.

The SRO must designate the Chairperson in order for that person to electronically sign the form in IAR. The SRO may designate multiple Reviewers to perform this task. Refer to How to Designate Reviewers.

IMPORTANT: Please keep in mind that Reviewers must complete the form while the meeting is still in the Edit Phase. Once the Edit Phase ends, Reviewers may no longer have access to the meeting, including links to the SRG Minutes/Budget Form.

To sign the SRG Minutes/Budget Form:

  1. Select the SRG Minutes/Budget Form link from the Action column of the specific meeting on the List of Meetings screen.

    The review committee information, meeting date/time, and names of the chairperson(s) and SRO are included on the form. In the middle of the form is a breakdown of what was reviewed in the meeting. This information is system-calculated and broken down by Activity Codes of all applications in the meeting.

    For the SRO, the I Certify button is displayed for electronically signing the form. Chairpersons accessing this form will see a similar button under their own signature line but will not see the button(s) for others.

    NOTE: Select the List of Meetings link to close the form and return to the List of Meetings screen without signing the form.

  2. Enter the Meeting Adjourned Time and the Meeting Adjourned Date.
  3. Select the I Certify button.

    When electronically signed, the signature line updates to display the following: Electronically signed by [SRO Last Name, First Name] via Internet Assisted Review on <Date and Time>.

How Do I Designate a Reviewer to Sign the SRG Minutes/Budget

SROs can designate a Reviewer(s) to electronically sign the SRG Minutes/Budget form. Only Reviewers who are designated can electronically sign the form and certifications. SROs can designate more than one Reviewer per meeting. The designated Reviewer does not have to be the Chairperson of the meeting.

ClosedTo designate a Reviewer:

Select the Designate link in the SRG Minutes/Budget Signee column for the specific Reviewer. The value of the column immediately updates to Clear. Select the Clear link to remove the designation.

Once the designated reviewer electronically signs the form in IAR, a date displays under the Clear link.

My Reviewer Does Not Have the 'I Certify' Button

Only Reviewers who have been designated to sign the SRG Minutes/Budget Form have the ability to certify the form. If your Reviewer expects to be able to do this and cannot, Verify that the Reviewer is listed on the Track COI Certifications page and is designated as a signee of the form. Refer to
